Falls Service

Access to Care hosts a ‘Falls Assessment and Intervention Service’. This is now fully established and welcomes referrals, direct from General Practice.
The service takes referrals for patients, who have had a fall at home, are unlikely to require a hospital admission and would benefit from a same day assessment for aids, equipment and short term support at home.
The team works closely with the relevant Neighbourhood Team, but has capacity to visit in pairs and lift patients, where necessary. Evidence, to date, suggests that this team is able to avoid admission more frequently that ambulance crews, attending similar cases. Where unavoidable, admission will be organized, directly, through Access to Care.
This service is staffed by additional mobile staff, over and above the core AtC team and is currently available seven days a week 10am to 6pm, though AtC will offer whatever support it can at any time.
